Would this kind of care make sense for my joint?
Some mornings, a hip catches before your stride feels even. The soreness may ease, then return during errands or work outside.
Relief reported for someone else's knee may not fit your hip. Their joint may also have less wear than yours does.
What did people in human studies actually feel?
Human studies mostly followed people with mildly or moderately worn knees. They tell us less about a hip, shoulder, tendon, or very worn knee.
When people improved, they reported less soreness or easier daily movement. A small change on a clinic form may not mean better sleep or walking.
Ask whether the study involved your joint and a similar amount of wear. If it didn't, the result may not fit you.
What happens during a joint visit?
The clinician asks where the joint hurts and when soreness began. You'll also discuss old injuries, current pills, and care that didn't help.
The exam checks motion, strength, swelling, and tender spots. An X-ray or another joint scan may be reviewed when the exam calls for it.
Then you can discuss likely relief, cost, recovery, and added soreness. The clinician should also explain why a procedure may not fit.
Which warning signs should not wait?
A very hot, swollen joint with fever can mean an infection. Get medical help that day rather than waiting on a routine visit.
Seek prompt help after a fall if the joint looks bent or won't bear weight. Sudden weakness or numbness near the sore area also needs quick care.
Call after any joint procedure if redness spreads or soreness keeps rising. Don't wait for worsening signs to settle on their own.

A Bayesian network meta-analysis of 48 Level I-II randomized trials (9,338 knees) with a minimum 6-month follow-up ranked the four commonest intra-articular injections. HA and PRP both significantly improved pain versus placebo; HA, PRP and BMAC all significantly improved function versus placebo. SUCRA rankings were PRP 91.54, BMAC 76.46, HA 53.12, corticosteroid 15.18 and placebo 13.70 - corticosteroid ranked barely above placebo at six months and beyond.

A network meta-analysis restricted to LARGE randomized trials (at least 100 patients per group; 57 RCTs, 22,795 participants, 18 intra-articular interventions) found treatment effects were consistently larger in the 35 high-risk-of-bias trials than in the 22 low/unclear-risk trials. In the main analysis excluding high-risk trials, triamcinolone had the highest probability of exceeding the minimal important difference at weeks 2 and 6; hyaluronic acid had no effect on pain (SMD -0.04, 95% CrI -0.19 to 0.11, 11 trials) but higher odds of dropouts due to adverse events (OR 2.01) and of serious adverse events (OR 1.86). The effects of 16 of the 18 interventions were smaller than the MID and most were consistent with placebo effects.

The AAOS third-edition clinical practice guideline on non-arthroplasty management of knee osteoarthritis contains 29 recommendations and explicitly highlights intra-articular corticosteroid, hyaluronic acid AND platelet-rich plasma as the areas where better research is still needed - including osteoarthritis characterisation, subgroup and severity stratification, control for bias, and cost-effectiveness analysis.

A network meta-analysis of 43 trials (5,554 patients) reached the OPPOSITE ranking to the more recent orthobiologic-favourable reviews: steroids ranked most likely to be effective for pain and function, with adipose MSC and multiple PRP ranked LEAST likely; single PRP, multiple PRP and adipose MSC did not produce a relevant reduction in pain or improvement in function versus placebo. The authors noted treatment-effect differences were small and potentially not clinically meaningful either way.

A network meta-analysis of 79 RCTs (8,761 patients) covering eleven injectables - autologous conditioned serum, BMAC, botulinum toxin, corticosteroid, HA, MSC, ozone, saline placebo, PRP, PRGF and stromal vascular fraction - found the top-ranked treatment CHANGES WITH TIME POINT: high-molecular-weight HA plus corticosteroid ranked first for WOMAC at 4-6 weeks and 3 months, while PRP ranked first at 6 months. This is the clearest demonstration that 'which injection is best' depends entirely on when you measure.

The BMJ meta-analysis of viscosupplementation pooled 169 trials (21,163 participants) and found clear evidence of small-study effects and publication bias. In the prespecified main analysis of 24 LARGE placebo-controlled trials (8,997 participants) hyaluronic acid reduced pain by SMD -0.08 (95% CI -0.15 to -0.02), equivalent to 2.0 mm on a 100 mm scale - far below the -0.37 minimal important difference - and trial sequential analysis showed there has been CONCLUSIVE evidence of clinical equivalence to placebo since 2009. Fifteen large trials showed a significantly higher risk of serious adverse events (RR 1.49).

The ESSKA-ORBIT European consensus on blood-derived orthobiologics graded 28 question-statement sets; only 9 of 28 had high-level scientific support. Three statements reached grade A: that there is enough preclinical and clinical evidence to support PRP use in knee OA; that clinical evidence shows effectiveness in MILD TO MODERATE knee OA (KL grade 3 or lower); and that PRP provides a longer effect than the short-term effect of corticosteroid with a safer profile. The panel regarded PRP as a valid and possible first-line injectable option for KL grades 1-3.

A meta-analysis of the PLACEBO arms of 73 double-blind trials (5,895 patients) quantified what a saline knee injection alone achieves: statistically and clinically significant improvement in pain, function and quality of life at 1, 3 and 6 months, with responder rates exceeding 50% at all three time points, peaking around 4-8 months and declining by 12 months. Placebo response was stronger in trials with more female participants and in more recently published trials.

FDA states verbatim of stem cell products, stromal vascular fraction (adipose-derived cells), umbilical cord blood, Wharton's jelly, amniotic fluid and exosome products: 'None of these products have been approved for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain.' The only FDA-approved stem cell products in the United States are blood-forming (hematopoietic progenitor) cells derived from umbilical cord blood, approved only for disorders of blood production, and there are currently NO FDA-approved exosome products.

FDA's patient and consumer notice on regenerative medicine states that it has received reports of BLINDNESS, TUMOUR FORMATION, neurological events, bacterial infections including life-threatening blood infections, unwanted immune reactions, and cells migrating and forming unintended tissue, following the use of unapproved regenerative medicine products - a list that explicitly includes stromal vascular fraction, amniotic fluid, Wharton's jelly, ortho-biologics and exosomes.
